Immunization Requirements

All incoming 7th grade students are required to turn in an updated immunization record that shows proof of recent Tdap and Meningococcal (MCV4).

For 7th grade: 1 dose of Dephtheria/Tetanus/Pertussis (Tdap) is required if at least 5 years have passed since the last dose of tetanus-containing vaccine.

For 7th – 12th grade: 1 dose of quadrivalent meningococcal conjugate vaccine is required on or after the student’s 11th birthday. Note: If a student received the vaccine at 10 years of age, this will satisfy the requirement.

A complete list of immunization requirements can be found at:

https://www.dshs.texas.gov/immunize/school/school-requirements.aspx

If you are providing an exemption form, an original copy of the form must be submitted. Requirements can be found at: https://www.dshs.texas.gov/immunize/school/exemptions.aspx

In situations where vaccinations have been started late or are more than one month behind schedule, information on catch up schedules can be found at: https://www.cdc.gov/vaccines/schedules/hcp/imz/catchup.html
